ID Card Request Form
All Honorable faculty members, officials and employees of UITS must wear ID card around neck during their stay in the Campus. Wearing of the ID cards would make the Campus safe for all. Any individual entering without ID card would be treated as undisciplined act and against the Code of Conduct of UITS.
When can I get my ID?
Newly admitted students do not need to apply for an ID Card once they have been officially admitted or registered.
Staff must be currently employed by the University and Faculty must have an active contract and HR process must be complete.

Do I have to have my card with me all of the time?
You should always have your ID Card with you while you are attending the University. Your card identifies you as a current student, staff or faculty member, and it gives you access and privileges to library, computer labs etc.
Why is there a charge to obtain a new ID Card if mine is lost?
There are internal costs for manufacturing University ID cards. The University does not charge you for your initial ID, however, any IDs that the University has to replace due to loss or damage is charged to the customer.
Lost or Stolen ID card
If your ID card has been lost or stolen and you can provide a crime reference number issued by the police, a new card will be issued free of charge.

When does my ID Card expire?
Your ID is valid for as long as you have an active relationship with the university. You do not need to renew your UITS ID annually.
